Autre justification: The research requires measurement of brain-derived phosphorylated tau 217 (BD p-tau217) in absolute concentration units with sufficient analytical sensitivity and specificity to enable application of established, clinically validated diagnostic cut-points. These cut-points have previously been derived and validated by the research team and are integral to the scientific validity, reproducibility, and interpretability of the ongoing research programme. A review of available commercial assay platforms confirmed that: The S-PLEX BD-Tau assay supplied by Meso Scale Diagnostics enables quantification of BD p-tau217 in absolute concentration units with the required level of analytical sensitivity. Alternative commercially available platforms either: Measure different analytes; Provide only relative signal outputs rather than absolute quantification; or Do not achieve the required analytical performance necessary for application of validated clinical thresholds. The use of non-equivalent assay platforms would require redevelopment and re-validation of established clinical cut-points, invalidate comparability with previously published cohort data, and impose disproportionate scientific and financial burden. Accordingly, no reasonable alternative or substitute exists that meets the combined technical and scientific requirements of the study. The absence of competition arises from current technical limitations within the commercial assay market and has not been artificially created by the contracting authority.
